Our Community Health Workers are ready to help!

The Fulton-DeKalb Hospital Authority Community Health Workers (CHWs) are committed to addressing the needs of the residents of Fulton and DeKalb counties around access to care, social determinants of health, & mental health issues. 

Working in partnership with other community stakeholders, The FDHA CHWs are available to educate and connect residents to the appropriate resources needed to navigate the Medicaid Unwinding process. Our goal is to ensure all residents maintain proper insurance coverage.
